I.        Terms and Conditions for the U.S. Bank Visa Buxx Card

A. By accepting or activating your U.S. Bank Visa Buxx Card (your "Card"), you agree to be bound by the terms and conditions contained in this U.S. Bank Visa Buxx Cardholder Agreement (this "Agreement") which govern the terms of your U.S. Bank Visa Buxx account (your "Account") and use of your Card. In this Agreement, the term Card includes any Visa Buxx Cards issued by U.S. Bank National Association ("U.S. Bank") which access your Account. "You" and "your" means the person(s), including an authorized teen child of such person(s), authorized to use the Card as described below, who have received the Card from U.S. Bank. Your child must be at least 13 years of age in order to be authorized to use the Card. "We," "us," and "our" mean U.S. Bank, our successors, affiliates or assigns. Please read this Agreement carefully and keep it for future reference.

B. The laws of the State of Ohio apply to this Agreement without giving effect to conflict of law principles thereof whose application may cause the law of another state to apply. This Agreement contains an arbitration provision (including a class action arbitration waiver). It is important that you read the Arbitration Provision Section carefully.

C. You shall not use your Card for, and we may decline authorization for, any illegal transaction or Internet gambling transaction.

D. We may change the terms of, or add new terms to, this Agreement at any time, with or without cause, and without giving you notice, in accordance with applicable law. Refer to www.usbank.com/visabuxx for the most current terms.

II.      Definition

Your Card is a proprietary form of Visa Debit Card and is not a credit card. In conjunction with your Card and the rules associated therewith, you may qualify to have a Card issued and monies funded into the Account accessed by your Card. Your Card accesses the Account we have opened for your personal use, and the personal use of your teen child who is authorized to use the Card to access the funds in such Account. Your Card does not access any other account. Your Account does not earn interest. Funds associated with your Account are FDIC (Federal Deposit Insurance Corporation) insured to the maximum allowed by law. The Visa Buxx Card program is offered by U.S. Bank under license from Visa and is subject to all the terms and conditions set forth in this Agreement, as they may be updated from time to time. Refer to www.usbank.com/visabuxx for the most current terms. You will notify us of the name, age, relationship and address of any teen child you authorize to use the Card and of any changes to such information.

III.     Using Your Card

A. You may use your Card to:

1. Pay for purchases anywhere Visa debit cards are accepted;

2. Perform transactions at Visa or PLUS® branded automated teller machines ("ATM");

3. Perform a cash advance at participating financial institutions; and

4. Access our telephone and online banking services listed below.

B. You may not transfer the cash balance available on your Card to another bank or card product.

C. Your Card is not transferable to another person and resale is strictly prohibited. Except as set forth herein, the person who purchases the Card is the only person authorized to use the Card. Do not give your Card to anyone else to use. U.S. Bank is not liable for the revocation of funds on Cards that have been resold.

D. You will be provided with a Personal Identification Number (a "PIN") which will allow you to use the Card at ATMs and for making purchases where entering your PIN is allowed. You can activate your Card and obtain a PIN by calling us at 888-314-2899. Your Card and PIN are provided for your use and protection, and you will:

1. not disclose the PIN nor record it on the Card or otherwise make it available to anyone else;

2. use the Card, the PIN and any ATM only as instructed;

3. promptly notify us of any loss or theft of your Card or PIN (See Section XIV); and

4. be liable for any transactions made by a person you authorize or permit to use your Card and/or PIN. If you permit someone else to use your Card, we will treat this as if you have authorized this person to use your Card and you will be responsible for any transactions initiated by such person with your Card.

E. You may load additional value onto your Card at any time before the expiration date on your Card, subject to certain restrictions set forth on the Fee Schedule included with your Card. Upon authorization, this will cause monies to be transferred into the Account which is accessed by your Card.

F. If you choose payroll direct deposit to your Card:

1. Your employer is responsible for transferring funds to U.S. Bank to load onto your Card. These funds will be transferred by your employer to U.S. Bank and loaded onto your Card by U.S. Bank according to the schedule agreed upon by your employer and U.S. Bank. U.S. Bank has no obligation to you in the event your employer delays in providing or fails to transfer funds to your Card.

2. Your employer may retain the right to deduct funds from the funds stored on your Card in order to correct a previous error or overpayment to you or for other reasons. You hereby authorize U.S. Bank to accept instructions from your employer to add or deduct funds from your Card and, in the case of a deduction, to return those funds to your employer. If you have a dispute with your employer about the amount of your wages, salary or other compensation, or the amount that the employer loads onto or deducts from the Card, you agree to not involve U.S. Bank in the dispute and to resolve that dispute solely with your employer.

3. You acknowledge and agree that except as set forth in this Section, your employer shall not be liable for any claims by you in connection with this Agreement.

G. Limitations on transfers will apply to your Card use as set forth on the Fee Schedule included with your Card. We reserve the right to change these transfer limitations without notice, which we make based upon periodic risk assessments.

H. Any bonus loads associated with a promotion will not be credited if the Account is closed within sixty (60) days after enrollment.

I. A check cannot be issued to refund any remaining balance on your Card. All balances must be removed by you via a purchase transaction, ATM withdrawal or teller withdrawal.

J. Be aware that some merchants (for example, restaurants, car rental agencies, salons, mail-order companies, cruise lines and pay-at-the-pump gas stations) temporarily require that your Card have an available balance greater than the purchase amount to ensure sufficient funds for tips or incidental expenses. The reason for this temporary difference is that the final amount of your purchase (including tips or incidental expenses) is not known at the moment when you or the merchant swipes your Card for authorization. Authorization tells the merchant whether your Card is legitimate and has enough money to cover your approximate final purchase amount. Only the actual amount spent will be deducted from your Card. The authorized amount is held (which means the authorized amount is not available for your use) until the transaction settles to your Account. Typical examples include:

1. Most restaurants and salons temporarily add a certain percent of your purchase (approximately 20%) amount to your bill to cover the tip. Make sure your balance can accommodate this additional percent amount or your transaction will be declined.

2. Hotels will verify that your Card has sufficient funds to pay an estimated bill. That amount will be held in your Account, making it unavailable for other purchases. When you check out, the hold will be removed and the actual bill amount will be deducted. You can avoid having funds held by using a Visa credit card at check-in. Your Card may be used to make the final payment.

3. Automated Fuel Dispensers and Pay-at-the-Pump purchases authorize for a pre-set amount which may be greater than your actual purchase amount. It is recommended that you pay inside and tell the operator how much you want taken from your Card to avoid confusion. When you pay at the pump the pre-set amount is automatically authorized and held until the actual transaction posts (typically within 8 business days). If you have less than the pre-set amount on your Card, your transaction will be declined at the pump even if your final transaction is less than the pre-set amount and you have enough funds to cover the final amount.

K. Preauthorized payments

1. Right to stop payment and procedure for doing so. If you have preauthorized payments with your Card, you can stop any of these payments. Here’s how: Call us at 888-310-2899 or write us at: U.S. Bank Visa Buxx Cardholder Services, P.O. Box 551888, Fort Lauderdale, FL 33355-1888, in time for us to receive your request three business days or more before the payment is scheduled to be made. If you call, we may also require you to put your request in writing and get it to us within fourteen days after you call.

2. Notice of varying amounts. If these regular payments vary in amount, the person you are going to pay will tell you, ten days before each payment, when it will be made and how much it will be. You may choose instead to get this notice only when the payment would differ by more than a certain amount from the previous payment, or when the amount would fall outside certain limits that you set.

3. Liability for failure to stop payment of preauthorized transfer. If you order us to stop payment three business days or more before the transfer is scheduled, and we do not do so, we will be liable for your losses or damages.

IV.     Overdrafts

Each time you use your Card, the amount of the transaction and any associated fees will be debited from your Card. Typically you cannot exceed the funded balance available on your Card. However, under certain limited circumstances you may be able to perform a transaction that exceeds the balance on your Card (an "Overdraft"). If this happens, you shall remain fully responsible for the amount of your purchase or withdrawal which exceeded the balance available on your Card, and you agree to immediately forward payment to us for all Overdrafts. We reserve the right to automatically debit Overdrafts from any subsequent credits to your Card.

V.       Use of Telephone Banking Services

You may obtain Account balances and review recent Card activity by calling [888-310-2899]. This information along with a 60-day history of Account transactions is also available online at [www.usbank.com/visabuxx.] You also have the right to obtain a 60-day written history of Account transactions by calling [888-310-2899], or by writing us at [U.S. Bank Visa Buxx Cardholder Services, P.O. Box 551888, Fort Lauderdale, FL 33355-1888]. A fee may apply. Your first two calls to Live Customer Service are free. Additional calls will be charged as described on the Fee Schedule included with your Card. You may call to report lost or stolen Cards or to dispute charges; these calls will not be charged a fee.

VI.      Expiration

Your Card expires on the expiration date shown on the front of your Card, except where prohibited or modified by applicable law. Your Card will automatically renew upon expiration if your Card and Account are active and in good standing.

VII.    Service Charges and Fees

A. You may be charged certain fees in connection with your Card and Account. We will charge you, and you agree to pay to us, the fees and charges (collectively, the "Fees") set forth on the Fee Schedule included with your Card. Fees are subject to change from time to time, and you will receive notice of Fee increases as required by applicable law. Fees will be automatically deducted from the balance in your Account. Without limiting the generality of the foregoing, you agree that we may deduct an inactivity fee as permitted by applicable law as set forth in the Fee Schedule included with your Card.

B. We may reduce certain fees to purchase your Card or perform value transfer to your Card if you are a U.S. Bank customer. You are a U.S. Bank customer if you use a U.S. Bank funding source (a U.S. Bank check card, credit card, checking account or savings account) to fund your Card.

VIII.   Verification of Transactions

A. You will have electronic access to an electronic transaction history describing each transaction and charge to your Account during the applicable period. This electronic transaction history will be made available until your Card has a zero balance. This electronic transaction history will reflect transactions and charges made in connection with your Card.

B. Your electronic transaction history will be available to you online in electronic format for viewing and printing at www.usbank.com/visabuxx.

C. You may choose to have a paper statement mailed to you for a fee as set forth in the Fee Schedule included with your Card.

D. You can get a receipt at the time you make any transfer to or from your Account using an ATM or point of sale terminal.

IX.      Failure to Complete Transactions

A. We are not liable for incomplete transactions due to insufficient funds in the Account. Please note the following timeframes for funding your Card:

1. If you fund your Account with a check card, credit card or by payroll direct deposit, the balance will be available immediately (barring technical problems);

2. If you fund your Account with an automated clearing house ("ACH") transfer from your checking or savings account, such funds will be available after three (3) to ten (10) Business Days; and

3. If you authorize a transaction and then fail to make a purchase of that item as planned, the approval will result in a hold for that amount of funds.

B. Neither we nor any other bank or business will be liable to you for failure to accept or honor the Card.

C. If we do not complete a transfer to or from your Account on time or in the correct amount according to our Agreement with you, we may be liable for your losses or damages. However, we will not be liable:

1. If, through no fault of ours, you do not have enough money in your Account to make the transfer;

2. If the ATM where you are making the withdrawal does not have enough cash;

3. If the terminal or system was not working properly;

4. If circumstances beyond our control (such as fire or flood) prevent the transfer, despite reasonable precautions that we have taken; or

5. Any other exceptions stated in this Agreement.

D. If you do not have enough money in your Account to complete a particular transaction, you may request that the merchant split your purchase amount between your Card and another form of payment, such as a credit or debit card or cash. If you are uncertain as to your exact balance, please log into our website at www.usbank.com/visabuxx or call Customer Service at 888-310-2899 to verify your balance prior to attempting to make a purchase.

X.       Returns and Refunds

You agree to settle all disputes about purchases you make using your Card with the merchant who honored the Card. If you need to return merchandise purchased with your Card you must work directly with the merchant from whom you purchased the merchandise. If you are entitled to a refund for any reason for goods or services obtained with your Card, you agree to accept credits to your Account in place of cash. Please review Section XV if there is an error on your Account.

XI.      Foreign Transactions

A. You may use your Card for retail purchases from foreign merchants and for cash withdrawals from foreign ATMs that bear either the PLUS System or Visa logos. Some merchant and ATM transactions, even if you and/or the merchant or ATM are located in the United States, are considered foreign transactions under the applicable Visa rules, in which case we will add the foreign transaction fees in this Section to those transactions. We do not control how these merchants, ATMs and transactions are classified for this purpose. The exchange rate in effect when the charge is processed may differ from the rate in effect on the date of the transaction or posting to your Account.

B. If you use your Card at a merchant or an ATM that bears the Visa logo (and no PLUS System logo), the charge will be processed through the Visa system and will be converted into U.S. Dollars according to the applicable rules established by Visa from time to time. If your transaction is processed through Visa, your foreign currency transaction will be converted to U.S. Dollars by multiplying the amount of the foreign currency times (i) a rate selected by Visa from the range of rates available in wholesale currency markets for the applicable central processing date, which rate may vary from the rate Visa itself receives, or (ii) the government-mandated rate in effect for the applicable central processing date. To the converted transaction we will add a foreign fee of up to three percent (3%) multiplied by the resulting dollar amount. We may assess the fee of up to three percent (3%) on all transactions in which the merchant is located in a country other than the U.S., even in transactions that do not require currency to be converted. If your foreign transaction is processed by Visa and is in or converted to U.S. Dollars prior to being processed by Visa, we will add a foreign fee of up to two percent (2%) multiplied by the U.S. Dollar amount.

C. If you use your Card at an ATM that bears only the PLUS System logo (and no Visa logo), the transaction will be processed through the PLUS System and will be converted into U.S. Dollars at the exchange rate established, from time to time, by the operator of that ATM. To the converted transaction we will add a foreign fee of up to three percent (3%) multiplied by the resulting dollar amount.

D. If you use your Card at an ATM that bears both the Visa and PLUS System logos, the ATM operator will determine whether to send your transaction over the Visa or PLUS System network using such network’s respective currency conversion rules then in effect, as explained above.

XII.    Amendment and Cancellation

A. We may at any time change or repeal these terms and conditions. You may be notified of any change in the manner provided by applicable law prior to the effective date of the change. However, if the change is made for security purposes or to comply with applicable law, we may implement such change without prior notice.

B. You agree to be notified by e-mail of any changes to this Agreement. Any changes will be included in this Agreement that may be accessed online at www.usbank.com/visabuxx. The e-mail notification and online posting of changes to this Agreement may be accessed on your home computer provided that you have access to the Internet, Internet browser software, and an e-mail account. Your computer must be equipped with a printer in order to print and retain paper copies of this Agreement.

C. We may cancel or suspend this Agreement or any features or services of your Card described herein at any time. Your Card remains our property and we may cancel your right to use your Card at any time. You may cancel this Agreement by returning your Card to us. Your termination of this Agreement will not affect any of our rights or your obligations arising under this Agreement prior to termination.

D. A copy of the current Agreement is available on our website, www.usbank.com/visabuxx.


A. Your Card and your obligations under this Agreement may not be assigned except to an authorized user who is an approved enrollee in the Program. We may transfer our rights under this Agreement.

B. Use of your Card is subject to all applicable rules and customs of any clearinghouse or other association involved in transactions.

C. We do not waive our rights by delaying or failing to exercise them at any time.

D. If any term of this Agreement is found by a court to be illegal or unenforceable, all other terms will still be in effect.

E. As used in this Agreement, "Business Days" are Monday through Friday excluding Federal holidays.

XIV.   Loss, Theft or Unauthorized Use

A. Tell us AS SOON AS POSSIBLE if your Card or PIN has been lost or stolen. You may be responsible for the unauthorized use of your Card if you fail to notify us promptly that your Card has been lost or stolen. If you believe your Card has been lost or stolen or that someone has performed a transaction using your Card without your permission or accessed or may access money from your Account without your permission, call us toll free 24 hours a day, 7 days a week at 888-310-2899 or write us at: U.S. Bank Visa Buxx Cardholder Services, P.O. Box 551888, Fort Lauderdale, FL 33355-1888. Telephoning is the best way to reduce the amount of your possible losses.

B. Consumer Liability for Unauthorized Transfers.

1. Zero Liability for All Transactions: Except as provided in the next paragraph, you are generally protected from all liability for unauthorized use of your Card, the associated Account number printed on it (including purchases made over the phone or online), or its associated PIN. You still need to report the loss or theft of the Card and any unauthorized transactions as soon as you can, however, to get any unauthorized transactions reversed, to prevent further unauthorized transactions, and to avoid liability for subsequent purchases we could have prevented had you given us notice. This policy also protects you from liability for other types of electronic funds transfers.

2. 60 Days after Notice: If you fail to give us notice of an unauthorized transaction within 60 days of the earlier of the date you electronically access your Account, if the transaction could be viewed in your electronic history, or the date we sent the first statement or transaction history on which the unauthorized transfer appears, you may not get back any money you lost after the 60 days if we can prove we could have stopped someone from taking the money if you had told us in time. If a good reason (such as a long trip or hospital stay) kept you from telling us, we will extend this time period.

C. If your Card or PIN has been lost or stolen we will close your Account and open a new one (and send you a new Card) to prevent additional losses.

XV.     Your Right to Dispute Errors

A. In case of errors or questions about your electronic transactions, call 888-310-2899 or write to U.S. Bank Visa Buxx Cardholder Services, P.O. Box 551888, Fort Lauderdale, FL 33355-1888, as soon as you can if you think your electronic transaction history or receipt is wrong or if you need more information about a transaction listed on the electronic transaction history or receipt. We must hear from you no later than 60 days after the earlier of the date you electronically access your Account, if the error could be viewed in your Account’s electronic transaction history, or the date we sent you the FIRST written history on which error appeared. You may request a written history of your Account at any time by calling us at 888-310-2899 or writing us at U.S. Bank Visa Buxx Cardholder Services, P.O. Box 551888, Fort Lauderdale, FL 33355-1888.

B. The following information must be contained in that notice:

1. Your name and Account number

2. Describe the error or the transaction you are unsure about and explain as clearly as you can why you believe it is an error or why you need more information;

3. The dollar amount of the suspected error; and

4. The date the transaction occurred.

C. If you tell us orally, we require that you send us your complaint or question in writing within ten (10) Business Days after speaking with us.

D. Generally, we will tell you the results of our investigation within ten (10) Business Days after we hear from you and will correct any error promptly. If we need more time, however, we may take up to forty-five (45) calendar days to investigate your complaint or question. If we need to do this, we will credit your Account within ten (10) Business Days for the amount you think is in error so that you will have use of the money during the time it takes us to complete our investigation. If we ask you to put your complaint or question in writing and do not receive it within ten (10) Business Days, we may not credit your Account, although we will still investigate your complaint or question. If notice of an error (i) involves new Accounts (that are open fewer than thirty (30) days), (ii) involves an electronic funds transfer that was initiated from a location other than in any state, territory or possession of the United States, the District of Columbia, the Commonwealth of Puerto Rico or any political subdivision of any of the above; or (iii) involves an electronic funds transfer resulting from a POS debit card transaction, the applicable time period for completing our investigation shall be ninety (90) calendar days in place of forty-five (45) calendar days. With respect to new Accounts (that are open fewer than thirty (30) days), we may take up to twenty (20) days to credit your Account for the amount you think is in error.

E. We will tell you the results within three (3) Business Days after completing our investigation. If we decide there was no error, we will send you a written explanation. You may ask for copies of documents that we used in our investigation. If we issued provisional credit to your Account and there is no error, the amount of that provisional credit will be subtracted from your Account. If you need more information about our error-resolution procedures, call us at the telephone number shown above.

XVI.   Disclosure of Account Information to Third Parties

We will disclose information to third parties about your Card or the transfers you make: (i) where it is necessary for completing transactions; (ii) in order to verify the existence and condition of your Account for a third party, such as a credit bureau or merchant; (iii) in order to comply with government agency or court orders; or (iv) if you give us your written permission. You will receive a copy of our Privacy Pledge with your Card mailing and at least once annually. We also post it on our web site at www.usbank.com. Our Privacy Pledge describes how we collect, protect, and use your confidential financial and other information and the circumstances in which we share your information with members of our corporate family and with unaffiliated third parties. The Privacy Pledge also tells you how you can: (i) limits the ways we share, or (ii) request corrections to the information we maintain about you.

XVII.  Arbitration

A. You agree that either you or we can choose to have binding arbitration resolve any claim, dispute or controversy between you and us that arises from or relates to this Agreement or the Card and Account (individually and collectively, a "Claim"). This does not apply to any Claim in which the relief sought is within the jurisdictional limits of, and is filed in, a small claims court. If arbitration is chosen by any party, the following will apply:


2. Arbitration will only decide our or your Claim, and you may not consolidate or join the claims of other persons who may have similar claims. YOU WILL NOT HAVE THE RIGHT TO PARTICIPATE AS A REPRESENTATIVE OR MEMBER OF ANY CLASS OF CLAIMANTS, OR AS A PRIVATE ATTORNEY GENERAL, PERTAINING TO ANY CLAIM SUBJECT TO ARBITRATION.

3. The arbitration will be performed in accordance with this Arbitration Provision and the rules of the chosen arbitrator in effect when the Claim is filed.

4. The arbitrator’s decision will generally be final and binding, except for the limited right of appeal provided by the Federal Arbitration Act.

5. Other rights that you would have if you went to court might also not be available in arbitration.

B. The party commencing the arbitration may select to use either JAMS or the American Arbitration Association ("AAA") (or, if neither of these arbitration organizations will serve, then a comparable substitute arbitration organization agreed upon by the parties or, if the parties cannot agree, chosen by a court of competent jurisdiction). If JAMS is selected, the arbitration will be handled according to its Streamlined Arbitration Rules unless the Claim is for $250,000 or more, in which case its Comprehensive Arbitration Rules shall apply. If the AAA is selected, the arbitration will be handled according to its Commercial Arbitration Rules. You may obtain rules and forms for JAMS by contacting JAMS at 800-352-5267 or www.jamsadr.com and for the AAA by contacting the AAA at 800-778-7879 or www.adr.org. Any arbitration hearing that you attend will take place in the federal judicial district where you reside. At your request, we will advance your filing and hearing fees for any Claim you may file against us. If you prevail on your Claim, we will pay your arbitration costs and fees, other than attorney, expert and witness fees and expenses. We will also pay any fees or expenses that applicable law requires us to pay. The arbitrator shall apply applicable substantive law consistent with the Federal Arbitration Act, 9 U.S.C. §§ 1 through 16, including but not limited to applicable statutes of limitation, and shall honor claims of privilege recognized at law. Judgment upon the award rendered by the arbitrator may be entered in any court having jurisdiction.

C. This Arbitration Provision shall survive termination of your Card and Account. Notwithstanding any language in this Agreement to the contrary, this Arbitration Provision shall be governed by federal law, including the Federal Arbitration Act. Notwithstanding any language of this Agreement to the contrary, should any portion of this Arbitration Provision be held invalid or unenforceable by a court or other body of competent jurisdiction, this entire Arbitration Provision shall be automatically terminated and all other provisions of this Agreement shall remain in full force and effect.

Foreign Transaction Fee

Up to 3% per transaction

A fee charged on a transaction in which the merchant is located in a country other than the U.S. or processes transactions outside of the U.S.



There is no charge to use your Card to make a purchase with your purchase at participating merchants. 

ATM Withdrawal



U.S. Bank National Association will not charge you a fee at any U.S. Bank-branded ATM.  If you use any other ATM, U.S. Bank will charge a fee to your account per ATM transaction.  Also, the owner of a non-U.S. Bank-branded ATM may assess a surcharge fee on any ATM transaction you complete. 

ATM Balance Inquiries


A fee for ATM balance inquiries.  Also, the owner of the ATM may assess a surcharge fee. 

ATM Declines


A fee for a transaction that is declined at an ATM. Also, the owner of the ATM may assess a surcharge fee. 

Cash Advance / Teller Withdrawal


A fee for a cash withdrawal from a teller at a financial institution. 

Card Initiation - Enrollment Fee

U.S. Bank Customer $10.00

Non U.S. Bank Customer $15.00

A fee will be collected at the time of purchase. You are eligible for the U.S. Bank Customer enrollment fee if you have a U.S. Bank funding source (check card, credit card, checking/savings account).

Value Transfer Fee

Non U.S. Bank Funding Source $2.50

U.S. Bank National Association will not charge you a fee when transferring money from a U.S. Bank funding source. If you use any other Non U.S. Bank funding sources, U.S. Bank will charge a fee to your account. There is no charge for value loads via payroll direct deposit.




If your Card Account is inactive for 90 days, a fee will be assessed to your Account on the first day of your next statement period and on the first day of each future statement period as long as your Account remains inactive. The fee will be assessed against the remaining value in your Card Account balance.  This fee will not be assessed if your Card Account balance is $0.00

Card Replacement


A non-expedited processing of a replacement or reissued Card.

Expedited Card Replacement


Expedited processing (2-3 business days) of a replacement Card.

Monthly Statement


If a monthly paper statement is requested, a fee will be assessed to your Card for each monthly paper statement. Statements accessed over the Internet are free of charge at www.usbank.com/visabuxx.

We reserve the right to change the above fee schedule upon written notification to you.
Additional fees from other parties may be incurred such as cell phone carriers or internet providers.

Transaction Limitations

A.  Limitations on frequency of withdrawals:

(i)   You may make only 3 cash withdrawals from an ATM per 24 hour period.

(ii)   You may make only 2 cash advance / teller withdrawals from a financial institution per 7 days.

B. Limitations on dollar amounts of withdrawals:

(i) You may withdraw up to $200 from an ATM per transaction.

(ii) You may withdraw up to $200 per transaction in the form of a cash advance / teller withdrawal from a financial institution.

(iii) You may withdraw up to $410 in the form of a cash advance / teller withdrawal from a financial institution per 7 days.

C. Limitations on dollar amount for Value Loads:

(i) You may load up to $500 per transaction.

(ii) Your maximum value on your Card is $2,000.

(iii) You may load up to $2,000 on your Card per 30 days. For purposes of fraud prevention and account security, there are other transaction limits (including transfer limits) that apply.
